Checklist/Documents Required for Re-registration/Validation under32 C of the Pharmacy Act, 1948:

  • 10th (D.O.B) Certificate (Self attested).
  • 10+2 Certificate, (Self attested) if any.
  • Category of Registration:
    Matric/Graduation with experience / Medical Assistant/Pharmacist/Compounders Diploma/ D.Pharmacy / B.Pharmacy Certificate (self-attested), whichever is applicable, in which she/he was previously registered in the repealed J&K Pharmacy Act, Samvat, 2011.
  • Pharmacist Registration copy and Last Renewal thereof (self-attested).
  • One Character Certificate certified from concerned SHO/ Sarpanch/ Corporator / Ist Class Magistrate. (Click here to Download Character Certificate Format)
  • Aadhar Card Certificate/ Number.
  • Address proof
  • Identity Proof.
  • One Pass port size Photograph with white background.
  • Email Id
  • Signature of the candidate.

  • License details if any.
